How the Haven partnership works
Two ways to partner, a flat 40% referral commission, and a short list of paperwork. Here is the whole picture in writing so you can evaluate it on your own time.
Wholesale, in-office
- Flat 50% margin: $67.50 per bottle wholesale, $135 retail. Same rate on the first order and every reorder.
- Low starting order, a couple of each, so you can see how it moves before scaling.
- 60-day buy-back guarantee on anything that does not sell.
- Patients leave with product in hand: better adherence and a real monthly revenue line for the clinic.
- Buy tax-exempt with a resale certificate on file (see paperwork below).
Referral
- A flat 40% commission on every referred sale.
- Your own affiliate link and promo code, with every sale attributed to you.
- Access to the affiliate kit, custom carts, and direct support.
- A good fit if you would rather not hold stock.
A flat 40%, from your first sale
Every referral partner earns 40% of referred sales. No tiers to climb, no thresholds to track. The rate on your first sale is the rate on every sale after it.
Wholesale partners make margin on stocked product rather than commission, so the 40% applies to the referral path. The W9 requirement below comes into play at $600 in commission, which is $1,500 in referred sales.

W9
Required before we can pay commissions once your referred sales cross $3,000 (about $600 in commission), per IRS reporting rules. Download the blank W-9, fill it in, and email it to accounting@havenwellness.com.
Tax-exempt resale
Reselling to patients in your office? You can buy wholesale tax-exempt with a resale certificate on file. Sales tax is run state by state, so the right form depends on where your clinic is. Send your completed certificate to accounting@havenwellness.com and we set your account to ship tax-exempt. Full detail below.
ACH for payouts
Referral commissions are paid by direct deposit. Send your bank details or a voided check to accounting@havenwellness.com and accounting will set up ACH. We will confirm your pay schedule during onboarding.
Tax-exempt resale, in detail
Because you collect sales tax from your patients at retail, your wholesale purchases from Haven can qualify for a resale exemption. We need a valid resale certificate from your clinic on file before your first tax-exempt order. Which form you use depends on your state:
- Utah clinics: Utah form TC-721, completed with your Utah sales tax license number.
- Most other states: the multistate MTC Uniform Sales and Use Tax Resale Certificate, completed with your own state's sales tax registration number. This one form is accepted as a resale certificate in 36 states.
- A few states (for example California and Florida) require their own state form instead. If that is your state, our accounting team will tell you exactly what to send.
Worth knowing: the certificate is specific to your clinic, so each clinic provides its own, signed, with its own registration number. You will need a sales tax permit in your state to complete it. One completed certificate covers your ongoing orders until it expires or your clinic's details change, so you do not need a new one for every order. Your state's tax department or your accountant can point you to the right form, and we are happy to confirm what we need.
